Late or Mid-Year Registrants

No student will be allowed to register with LEMI education partnership or any of their Utah charter schools, in any semester, after the deadline set for the 2013-2014 school year.

Existing Students - Previously enrolled for 2012-1012 school year.
1.  Fill out and send in Registration Part 1 and 2.
2.  Parent signs Signature Verification and send it in (On last page of Registration
       Part 2.)


New Students:
1. Fill out and send in New Student Registration Parts 1 and 2.
2. Parent signs Signature Verification card in (On last page of Registration Part 2.)
3. Send in a Birth Certificate and Immunization or exemption form.
